The ptr/xptr ref/xref pairs are slated for major revision in
P5. The suggestion currently on the table is to remove
xptr/xref, and give ptr/ref an attribute to point to a URI.
This would make it much more like the HTML <a> element. How
you point to "third paragraph from the sun, turn left, find
the first bibl" in the external document is less clear
(though the working group has proposals). Most people I
suspect will use the simple #NAME mechanism which is
familiar from HTML
