question for the group:
as i understand, most peds em and em physicians follow the guideline of
working up and admitting any febrile infant less than 4 weeks of age.
most of us also follow the rochester criteria (or any of the other
well-validated criteria) to decide which febrile 4-12 week olds can go
in talking around the country, it seems that that second group of
infants is where folks most differ in their workups. some follow the
guidelines for only 4-8 weeks, others follow it for 4-10 weeks, others
for 4-12 weeks. i've heard of a few following it only for 4-6 weeks.
my question is: is there any newer literature to support the idea of
narrowing the previous recommendations of 4-12 weeks to a smaller
group?? i have done a pretty good search and can't find much.
i'd be interested to know if any ED's have written policies on
narrowing this second group and what literature you used to support
