From memory, the last time I did something like that I used XSLT2 and
xsl:for-each-group, which you can then group the text into discreet units based
on the <lb/> elements (and can then nest to provide other layers of hierarchy).

Seems a lot easier. I would do a combination of the descriptive markup
suggested by Ralph, and <seg>, if I were doing it.
