I understand the concern. I would refer you to the large (N=1,500) prospective cohort study by Keri Carstairs et al from the Naval Medical Ctr. ED in San Diego. The manuscript that was published in the June 2007 issue of Ann Emerg Med suggests that even one dose appears to be protective against bacteremia (or IPD). 817 patients were < 1 yr i.e. received < 3 doses of Pneumovax. 400 had received only one dose.

I worry a little about extending this practice (of not obtaining blood cultures) down to 2 months. Infants are not fully immunized againt pneumococcus until 6 months so theoretically they have a higher risk of pneumococcal bacteremia prior to full immunization. On the other hand, they have maternal antibody offsetting this risk.
