In this hypothetical case---------Do you think a CXR would have made a difference in the case of a 27 week ex premie, now 7 months old (adjusted age 5 months)) who was being followed by cardiology for a small to medium PDA but deemed stable for a three month follow up. Pt was seen in the ED c/o coughing and vomiting once after feeding. He was treated with albuterol. An RSV was sent which was negative. Two hours later the wheezing cleared and pt feed well and was dc'd home. He then expired 56 hours after the ED visit. Some of my colleagues think a CXR would have made all the difference in the care........any thoughts??
Thanks so much.
