I had two infants with similar presentation
One got topped - neg
One did not (resulted in mild anxiety on my part for the full night)
But did extremely well
this is a well-known phenomenon and I'm sure that other people saw it as well

The child looked great (smiling and cooing), and had no other source for fever on exam but interestingly had a full fontanelle (not bulging but full and tense) and this was one of the presenting complaints. I vaguely recall reading somewhere that you can see transient bulging of the fontanelle due to vaccines. I did an internet search and pulled up some articles on the topic. One of the articles had a series of cases and stated it can last for 3 days on average. I did not do a CT or LP but instructed the family to return if the fever and/or bulging fontanelle persisted.
