With patients given Zofran to go home, I've had a patient who turned out to have new onset diabetes return in severe DKA with return delayed because the vomiting stopped and they waited until the patient was very sleepy. Another family wanted to give all of the zofran they were prescribed to make sure it wasn't working and the child returned with perforated appendicitis. In the phenergan days I had a patient come in with bacterial meningitis and the only reason they brought their severely ill child in was that they ran out of suppositories.I'm leery of antiemetics for home. We often see patients very early in their illness. We can give the best discharge instructions and still have the family fail to return.
