At the two CHOA facilities my group covers (Egleston and Hughes Spalding), charts on all admissions and transfers, and all charting by residents and fellows, must be completed before they leave after their shift. All other attending charts must be completed within 24 hours.
Those who are recurrent offenders may be put on a monitoring program and the really bad ones can have their privileges suspended (and have to pay out of pocket for reinstatement).
There are both medicolegal and reimbursement issues for quick chart completion.
