We're considering using the Step-by-Step approach for evaluating febrile infants >28 days of age. Step-by-step uses procalcitonin to identify high-risk infants. The sensitivity of procalcitonin is lower if the patient presents with fever of short duration (<2-4 hours). We're wondering what to do for these infants.
Does anyone use the step-by-step approach? If so do you observe infants and/or repeat procalcitonin if the fever started <2 hours ago?
