Dear Emergency Department Provider,

We are conducting a survey to explore the teaching practices of Emergency Department providers when prescribing EpiPens to children. If you prescribe EpiPens to children, please complete this survey to help us better serve this population. Your participation is entirely voluntary and we do not ask for any identifying information. The entire survey should take no more than 5 minutes of your time. When filling out the survey please limit your responses only to EpiPens, and do not include other epinephrine injection devices.
Your opportunity to practice emergency medicine will not be affected in any way by your choice to participate in the study or not. If you choose to participate, your consent is implied by completion of the survey. This study was reviewed by the Northwell Health IRB under IRB number 18-0893 and deemed exempt.
