Thanks for your reply.. But I still have some unresolved issues pertaining
to the approach of the febrile infant. I also wonder why you feel so
strongly about the age cut off being 2 months as opposed to 28days which
other authors reccomend.
SO just to play Devil's advocate,, why 2 months and not 3 months as was once
proposed? Wasn't Roberts and Borzy study done on kids under 12 weeks?
I have read most of the literature including your article in NEMJ, but dont
necessarily agree with statement that every 2 month old with Temp of 38
needs a complete swu.. Aren't there babies who get the common cold who can
be culled out of this group? What about those with gastroenteritis or RSV?
Isn't it an abuse of our authority to subject every babe <2 month to LP and

to apply some judgement in selecting which child undergoes a full work up?
Does the family milieux play some role in the decision? What if several
family members are afflicted with viral illnesses?

use 38.2 as their cutoff?
