In many cases, this is true.  In many languages a glottal stop [?] or
some other separator is inserted to keep them distinct.  Others may turn
into diphthongs, then there's the Spanish method of running them
together where "Santa Ana" comes out something like [santa:na].

For those that think consonant pairs are hard to pronounce, try looking
at Hawaiiian or some other Polynesian languages and see how difficult it
can be to pronounce some of the vowel combinations that occur.
