I generally use 10 mg/hr of albuterol continuously for asthma and don't
adjust for size except in the smallest infants. I figure they do a lot
of titrating of dose themselves through lower minute ventilation thus
more med going into the room in small children with low minute
ventilations.
 
I have used epinephrine in some kids with bronchiolitis when albuterol
failed with moderate success.
