Have had one horrific experience with bouginage when 2nd yr surgery
resident attempted procedure (without my knowledge) on 12 month old with
ingested coin in cervical esphagus. He had never done procedure before
and got verbal guidance from attending peds surgeon over the phone. Also
sedated pt with IM versed and morphine. Passed bougi into
trachea--patient aspirated and had repiratory arrest--which is where I
became involved as he was making repeat attempts to intubate patient--no
monitoring in place. Child survived after stay in PICU and attending peds
surgeon did not feel that this was out of line!!!
